Every person’s healing journey is unique, and continuity of care ensures that they receive the initial interventions they need to stabilize and follow-up treatment that can support their long-term wellness.

For example, a person who completes residential care no longer requires live-in care, yet they may not be prepared to return to their daily life without the support of a professional. In this case, outpatient services such as a partial hospitalization program (PHP) or an intensive outpatient program (IOP) can be an ideal progression in care. In PHPs and IOPs, individuals can engage in structured yet flexible programming that promotes their continued success.

Fostering Long-Term Stability

The path to lasting wellness is nonlinear and may be characterized by significant personal growth as well as setbacks. Sustained well-being requires a person’s concerted effort, a strong support network, and, oftentimes, progressively less restrictive professional care. A full continuum of care promotes long-term stability by offering consistent support and resources individuals can utilize throughout their healing process.

PHPs and IOPs alike provide a setting in which individuals can develop coping skills and receive real-time feedback to help them navigate life’s challenges and manage stressors. The Substance Abuse and Mental Health Services Administration (SAMHSA) reports that IOPs can help increase abstinence rates, decrease substance use disorder symptom severity, and reduce a person’s frequency of substance use.

We meet our patients where they are on their journeys to better health, providing intensive residential care that can be followed by PHP or IOP participation. Our partial hospitalization program is available for women age 18 and older and features supportive housing options that can benefit those who do not have a safe living environment to return to after completing residential care. This daytime treatment option incorporates group counseling, community outings, and experiential therapy to provide women in need with a well-rounded outpatient experience that is both structured and less intensive than residential care.

A progressively lower level of care, the intensive outpatient program can be utilized as a step-down from residential care or the PHP. Our IOP uniquely offers supportive housing options and flexible scheduling, with day and evening sessions available. IOP participation can bridge the gap between live-in care and traditional outpatient services, facilitating a smooth transition back to daily life.

Those who do not meet the criteria for our outpatient programs still receive comprehensive aftercare plans to support their ongoing success and reintegration into the community. Detailed continuing care plans may include referrals to a lower level of care and local Chicago support groups. Incorporating community resources and other Chicago outpatient services for women into each person’s aftercare plan can foster a network of support that extends beyond our formal treatment setting.
